While CAQH is common in the medical community, some Registered Dietitians may not have a compete CAQH. Below is our guide to support RDs completing their CAQH.

1. Gather Your Forms

• [ ] CDR Number (here) - you will input this as your state license number for ALL CDR STATES
• [ ] NPI (register for NPI Here)
• [ ] State medical license(s)
• [ ] Resume (with education dates & institution, training, internships, & work history (since graduation or last 10 years)
  • [ ] [REQUIRED] include explanation for any resume gaps +6 months

1. Confirm with your Employer (Bridge Partner)

• [ ] Who should be listed as Credentialing Contact (Name, Phone, Email)
• [ ] Appropriate Professional Liability Insurance & Certificate of Insurance Document
